Add a Card from the Voice Page QR Code

Accessing the QR Code

  • Navigate to the Today Page, Voice Page, or Kiosk Mode.

  • Locate the QR code icon in the bottom right corner of the screen

  • Click the QR Code icon labeled “tap to expand”

  • Scan the expanded QR code using your smart device camera

  • Open the submission link that appears

You may also access the QR code:

  • From a printed poster displayed around your unit

  • From a shared or emailed PDF

Submitting a Voice Card (Logged In Users)

After scanning the QR code, log in to LENS for full functionality.

You can:

  • Enter a title and description for your Voice Card

  • Create a due date

  • Add labels

  • Assign a reporter

  • Add team members

  • Upload attachments

  • Choose to submit the card anonymously if needed

To submit your card:

  • Click the Create button

  • View the confirmation message showing which board’s Voice Page received the card

Submitting a Voice Card (Not Logged In)

If you choose not to log in, you can still submit a Voice Card.

You can:

  • Enter a title and description

  • Create a due date

Limitations:

  • Cannot add labels

  • Cannot assign a reporter

  • Cannot add team members

To submit your card:

  • Click the Create button

View the confirmation message showing which board’s Voice Page received the card

After Submission

After submitting a Voice Card:

  • You can log in to LENS later from a desktop

  • Open the submitted card

  • Update details or formatting as needed

When you log in and update the card:

  • Your name is added to the card history

  • This provides visibility and accountability for follow-up

Why Use the QR Code Workflow?

Using the QR code allows staff to:

  • Quickly submit issues in real time

  • Access a mobile-friendly submission experience

  • Avoid limitations of SMS-based workflows

  • Choose between anonymous or fully attributed submissions
